APPOINTMENTS/RE-FILL REQUESTS
To make an on-line appointment or request a re-fill of your medication you must be an existing patient and have already filled out and turned in an enrollment form.
On the other hand, existing patients must call the office to make an appointment for:
Any visit for a child under 2 months
The first evaluation for ADD or ADHD
Pre-op appointments
Injuries (lacerations, possible broken bones, sprains, etc.)
Respiratory distress
It is our practice policy to accommodate all sick children that need to be seen the same day. If your child is ill and you cannot find an appointment on-line, please call the office for an appointment.
Please note: if you are scheduling a one-year or a four-year physical for your child, your child must have already reached their first or fourth birthday. If not, you will be asked to reschedule.
*In order to obtain a log-in to use the patient portal please fill out an enrollment form and turn it in to the office. The form can be obtained on-line in the Forms section of our website or in the office. Each child must have their own log-in.
